MyCareProfile.org will provide a safe, secure place to store and access important healthcare, home care, and long term care information.

Information, such as doctors and healthcare providers, medications, medical history, insurance providers, locations of important legal documents, needs to be accessible.

 

MyCareProfile.org will be a one-stop place to organize and easily update all of your care information. The site will be password protected, and therefore confidential. Only you and the friends/family you share your password with can access your information. LifeVine pledges to treat your information with the utmost of confidentiality. When it is launched, the tool will be a public service and free to anyone who wishes to use it.

 

MyCareProfile.org will consist of

 

  • My Healthcare (doctors/healthcare providers, preferred pharmacies, preferred hospitals, labs, medications, medical history, insurance providers)
  • My Home Care (household instructions for caregivers, diet/nutrition preferences and instructions, daily hygiene needs, pet care instructions, local businesses and points of service such as grocery stores, drug stores, dry cleaners, restaurants, etc.)
  • My Long-Term Care (preferred long-term care facilities and/or in-home services, critical care directives, locations of vital legal documents)
  • My Afterlife Care (funeral plans, burial provisions, locations of vital legal documents, spiritual concerns)
